• Deliver skilled, compassionate nursing care in patients’ homes and facilities.
• Conduct comprehensive assessments and develop individualized care plans that honor the patient’s wishes.
• Provide education, guidance, and emotional support to families and caregivers.
• Coordinate care with an interdisciplinary team to ensure seamless communication and holistic support.
• Administer medications, treatments, and symptom management with excellence and grace.
• Participate in on-call rotations, ensuring patients always receive the care they deserve.

• RN license (required) and hospice, palliative, geriatric, or home health experience (preferred).
• A calm presence, strong clinical judgment, and a compassionate heart.
• The ability to work independently while collaborating with a dedicated team.
