• Conduct timely, comprehensive admission assessments for new hospice patients in their homes, facilities, or hospitals.

• Educate patients and families on hospice philosophy, services, and plan of care
  , ensuring understanding and comfort.

• Collaborate with physicians, intake, and interdisciplinary team members to ensure seamless, coordinated transitions to hospice care.

• Complete documentation and orders accurately and efficiently
  , ensuring compliance and continuity of care.

• Provide emotional support, compassionate communication, and reassurance during the admission process.

• Represent Seasons Hospice with professionalism, empathy, and warmth — every single time.

• Current RN license in the State of Oklahoma.

• Hospice, palliative, or home health experience preferred (but not required).

• Excellent communication, assessment, and teaching skills.

• Strong organizational abilities and comfort working independently in the field.

• A calm, compassionate presence that brings comfort to patients and confidence to families.
