Salary Range: $84,000 – $86,000 DOE

• Deliver skilled, compassionate nursing care in patients’ homes and facilities.

• Conduct comprehensive assessments and develop individualized care plans that honor the patient’s wishes.

• Provide education, guidance, and emotional support to families and caregivers.

• Coordinate care with the interdisciplinary team to ensure seamless communication and holistic support.

• Administer medications, treatments, and symptom management with excellence and grace.

• Participate in on-call rotations, ensuring patients always receive the care they deserve.

• Active RN license (required).

• Hospice, palliative, geriatric, or home health experience preferred.

• A calm presence, strong clinical judgment, and a compassionate heart.

• Ability to work independently while staying connected to a supportive team.

• Exceptional communication, documentation, and organizational skills.
