Responsibilities

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside—including private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psychosocial assessments, interventions, and knowledge of community sources. The RN may work in Case Management, Admissions, or a Resource role.

Qualifications
  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
  • Works well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Works closely with patients on a 1‑to‑1 level while receiving strong support from a patient care team
  • Can work independently on a day‑to‑day basis, handling phone, email, written communication, meetings with healthcare professionals, and understanding hospice regulations
  • Strong time‑management skills to schedule visits, documentation, meetings, and prioritize urgent needs
  • Ability to learn an electronic medical records system
  • Must be willing to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area
