• Help develop and implement a facility-wide Antibiotic Stewardship Program based on nationally recognized guidelines to monitor and improve antimicrobial use.
• Document all stewardship program activities, both written and electronic.
• Communicate and collaborate with medical staff, nursing, laboratory, pharmacy, and hospital leadership, as well as infection prevention and control and QAPI programs, on antimicrobial use initiatives.
• Provide competency-based training and education to hospital personnel and staff, including medical staff and contracted personnel, on practical applications of stewardship guidelines, policies, and procedures.
• Review providers’ orders for appropriateness of drug, dosage, route, and potential interactions; counsel nurses and providers about medications and medication-related issues.
• Provide pharmaceutical information and assistance to all members of the patient care team to achieve departmental and organizational objectives.

• OK Pharmacist License
• BLS
• PGY 1 Pharmacy Residency and/or BCPS Certification within 3 years from date of hire.
