Location: Tukwila, WA 98168
Shift: Day Shift | 5x8-Hour (08:00 AM – 04:30 PM)
We are seeking an experienced Licensed Master Social Worker (LMSW) for a Home Health setting in Tukwila, WA. The ideal candidate will have recent home health or geriatric experience, strong communication skills, and the ability to manage patient care across a flexible territory.

Geriatric Patient Population
Provide comprehensive psychosocial assessments and interventions for geriatric patients in home health settings.
Develop and implement individualized care plans in collaboration with interdisciplinary teams.
Ensure timely and accurate documentation in the EMR system (HCHB).
Communicate effectively with patients, families, and healthcare professionals.
Manage caseloads efficiently, averaging 5–7 patients per day (to be confirmed during interview).
Maintain flexibility in covering assigned territory (up to a 60-mile radius).
Active LMSW license (license must be in hand).
Minimum 2 years of recent LMSW experience (home health preferred).
BLS certification required.
Experience with geriatric patient populations.
Strong documentation and communication skills.
Ability to work independently in a home health environment.
