• Implements and monitors patient care plans. Monitors, records, and communicates patient condition as appropriate. Serves as a primary coordinator of all disciplines for well-coordinated patient care. Notes and carries out physician and nursing orders. Assesses and coordinates patient"s discharge planning needs with members of the healthcare team. Conducts assessments using a systematic approach and analyzes data about the patient, considering physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
• Initial assessment
• Re-assessment
• Focused assessment based on patient condition, setting, and population
• Problem identification and planning/outcomes – sets measurable goals in collaboration with patient and family; documentation is included in the patient care plan
• Establishing daily goals with the patient and family; formulating a plan of care; transition or discharge planning
• Implementation of nursing care according to the care plan; ensures continuity of care and documentation in the patient’s record
• Evaluation of patient status and effectiveness of nursing care; care coordination with the healthcare team; delegation and supervision as needed

• Education:
  
  Graduate from an accredited School of Nursing
• License and
  
  Certifications:
• Basic Life Support (BLS) - American Heart Association
• Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days

• Education:
  
  Bachelor of Science in Nursing
• Certifications:
  
  Advanced Cardiac Life Support (ACLS) - American Heart Association, Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP) or others as required by individual department
