Overview

Responsibilities- Provide comprehensive outpatient primary care for all ages, conducting assessments, diagnosing, managing acute and chronic conditions, and delivering preventive care and health promotion.
- Work independently, maintain accurate EMR documentation, collaborate with staff, support quality improvement, and participate in ongoing education.
- Active AZ NP and RN licenses
- DEA
- BLS
- Minimum 5 years clinical experience
- Proficiency with EMRs
- Experience in community/rural health
- Bilingual skills
- Familiarity with value-based care
