Job SummaryProvides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.
Responsibilities- Implements and monitors patient care plans; monitors, records, and communicates patient condition as appropriate.
- Serves as primary coordinator of all disciplines for well‑coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient’s discharge planning needs with members of the healthcare team.
- Assessment:
- Initial assessment
- Re‑assessment
- Focused assessment based on patient condition, setting and population
- Problem Identification:
Nurse’s clinical judgment about the patient’s response to actual or potential health conditions or needs. - Planning and Outcomes:
- Establish daily goals with patient and family
- Formulate plan of care
- Transition or discharge planning
- Implementation:
- Ensures patient safety
- Medication administration
- Provision of ordered treatments
- Patient/Family education
- Provision of nursing care
- Documentation in healthcare record
- Evaluation:
Continuously evaluate patient status and effectiveness of nursing care and modify care plan as needed. - Care Coordination:
- Delegation
- Supervision
- Prioritization
- License
Required:
Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days. - Certification
Required:
Basic Life Support (BLS) – American Heart Association. - Certification Preferred:
Advanced Cardiac Life Support (ACLS) – American Heart Association, Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP) or others as required by individual department.
- Required:
Graduation from an accredited School of Nursing. - Preferred:
Bachelor of Science in Nursing.
