Responsibilities

In addition to performing visits and coordinating client care, the RN Home Health is the point of contact for all disciplines involved in patient care and oversees visit frequency for the episode. The RN consults with the physician and office as needed to detail patient care. Key responsibilities include accurate OASIS collection, maintaining current medication profiles, reporting lab values to the physician in a timely manner, and attending weekly case conferences and monthly case manager meetings.

Qualifications

• Be currently licensed as a Registered Nurse (RN) in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrate knowledge and skill in current nursing practice
• Possess a valid state driver’s license and automobile liability insurance
