Street Outreach Case Manager

• Connect with and motivate people living on the streets toward self‑sufficiency.
• Assist homeless people in navigating the social service system to identify resources that help end their homelessness.
• Set goals and develop an individualized case plan with each outreach prospect.
• Establish and maintain relationships with community stakeholders and attend community outreach events.
• Ensure client information is entered timely and accurately in the PRM database.
• Lead or participate in devotions, prayers, or other organization activities that promote a credible Christian witness and lifestyle.

• Bachelor’s degree in Social Work, Sociology, Psychology,, or another social service field, or 4 years of case management experience, or a combination of education and experience.
• Certified in Peer Support, preferred.
• Current valid Arizona driver’s license with a clean driving record of 2+ years and ability to qualify for company insurance policy.
• Knowledge of the problems facing the street homeless population and awareness of the resources that help people exit homelessness.
• High level of composure and ability to remain calm in stressful situations.
• Discrete and understand importance of keeping client information confidential.
• Excellent communication skills.
• Proficient in Microsoft Office apps.
• Solid and credible Christian witness and lifestyle that exemplifies Christ in character and actions.

Reliable transportation and Monday‑Friday 8:00‑16:00 hours required.

Salary: $18.00 – $25.00 per hour
