Field Nurse Practitioner

Overview

Assist high-risk, high-need Medicaid and AHCCCS members in their homes to improve health outcomes through care coordination, education, and treatment planning.

• Conduct patient examinations, review records, and create integrated treatment plans.
• Prescribe and reconcile medications, document encounters, and educate patients and families.
• Collaborate with healthcare providers and team members to ensure seamless care and transition management.
• Engage members in goal setting and address barriers to health in their home environments.
• Manage approximately 5 patients daily, overseeing a caseload of 100 members monthly with team support.

Requirements include a valid Nurse Practitioner license, experience in primary and chronic care, excellent communication skills, and the ability to connect with diverse patient populations. Bilingual in Spanish is preferred.

Position covers the Yavapai East area, offers starting salaries of $130,000+, with benefits such as insurance, 401(k), bonuses, and paid time off. Work involves on-the-road visits, with a focus on community-based, patient-centered care.
