Case Manager for Individuals with Disabilities
Full Time
Pay: $50,000 per year

- Case Managers coordinate in-home and community-based services for the elderly and people living with all types of disabilities.
- Working Mon-Fri 8a-5p. The position may occasionally require a slightly different schedule to accommodate client needs, including direct care shifts, some evenings or weekends.
- Performing on-call duties at least 4-5 times per year, including answering and managing emergency phone calls from 7p-7a M-F and all day Sat and Sun; duties may include direct care shifts.
- Maintaining and growing a caseload
- Scheduling shifts for clients and direct care staff
- Orienting and training direct care staff with clients
- Monitoring direct care staff performance
- Holding staff meetings as needed and attending weekly department meetings
- Conducting intake of new clients
- Meeting with clients/families in their home minimally every quarter
- Attending meetings with other agencies involved in a client's care
- Ensuring that all paperwork in client files is updated regularly
- Documenting all significant conversations and/or correspondences with staff and clients
- Maintaining client and agency confidentiality at all times
- 18 years of age or older
- Ability to obtain a current CPR/FA certification
- Ability to obtain fingerprint clearance
- Ability to clear a DES background check
- Ability to obtain a current T.B. clearance card
- The ability to work in a team environment
- The ability to handle difficult situations with clients/families
- The ability to work a flexible schedule
- The ability to learn and operate UCPSA program software
- The ability to work independently
- The ability to lift and transfer clients (up to 50 lbs)
- Own/have regular access to and be able to operate a motor vehicle for visiting and transporting clients, attending meetings outside of the office, etc. Proof of current registration and insurance of the vehicle must be kept on file with UCPSA
- At least one year of direct care experience, personally or professionally, a degree in a related field, and/or case management experience
