Care National is seeking a dedicated Nurse Practitioner / Physician Assistant to join our team in Arizona. This role is part of an innovative primary care model designed to deliver affordable, convenient, and personalized care.

• Evaluate, diagnose, and treat patients following standards of care.

• Coordinate patient services including referrals, hospital/SNF coordination, DME, and home health.

• Lead and participate in daily care team huddles.

• Complete medical record documentation promptly.

• Collaborate with clinical leadership on quality of care, outcomes, and best practices.

• Represent Prosano in the community by building relationships with healthcare and community partners.

• Flexible schedule including morning, evening, or weekend shifts.

• 100% on-site with potential to move between care centers.

• Graduate of accredited NP or PA program with Board Certification.

• Active, unrestricted NP or PA license in Arizona.

• DEA license, BLS certification, and AZ fingerprint clearance card required.

• 2?5 years of clinical experience preferred.

• Strong communication and teamwork skills.

• Commitment to value-based care and exceptional patient experience.

• EMR proficiency and basic computer skills.
