Summary- Position Type:
Full-time, non-exempt, hourly supervisor role. - Scope of Role:
Supervises a single program or unit, typically supporting 4‑8 individuals in residential settings or 10‑15 individuals in periodic, vocational or day programs. Duties are split between direct support, program activities, and supervision. - Key Responsibilities:
Supervise Direct Support Professionals, oversee day‑to‑day operations, implement person‑centered plans, manage health care, customer engagement, regulatory compliance, and physical environment oversight. Generally works 8‑16 flexible hours per week in a supervisory capacity and provides direct support for the remaining hours.
- Direct Support:
Works scheduled shifts as a direct support professional. - Records:
Documents as required; reviews individual records, billing and other program documentation daily for accuracy and completion. - Program Planning:
Assists the Program Director in developing individual program plans; trains and monitors implementation. - Team Meetings:
Attends and assists with routine progress meetings. - Compliance:
Assists the Program Director in maintaining licensing compliance; implements Sevita compliance plan; manages individual rights implementation. - Management of Individuals’ Assets:
Assures safety of individual funds and property; monitors spending and reconciles financial accounts monthly. - Incidents:
Documents incident reports; submits initial reports to the Program Director. - Billing and Utilization:
Compiles census or attendance information; reviews data, attests to accuracy, and submits accurate data to the appropriate billing authority.
- General Health Care:
Monitors health, documents, and communicates with medical professionals. - Appointments:
Schedules or assists with scheduling health care appointments; accompanies individuals as needed. - Medications:
Accurately administers or supports self‑administration, documents delivery, reviews medication documentation, and maintains medication security. - Medical Supplies and Equipment:
Purchases and maintains supplies and equipment in consultation with the Program Director. - Dietary Planning:
Develops menus based on individual diets and preferences;, overseeing purchasing and food service consistent with plans.
- Communications:
Maintains professional relations with individuals, families, case managers and other stakeholders. - Customer Service:
Implements Sevita Customer Service Standards; trains associates.
- Recruitment and Hiring:
Assists with recruitment; interviews and hires DSPs. - Employee Scheduling:
Populates and maintains schedules to ensure coverage and minimize overtime. - Training:
Assists onboarding and delivers ongoing training concerning individuals served and program policies. - Evaluation:
Prepares and conducts annual evaluations for DSPs in consultation with the Program Director. - Discipline and Termination:
Coaches employees; may remove or recommend removal of employees in emergency situations. - Employee Safety:
Responsible for employee safety and workers’ compensation within the program. - Management:
May assist or lead monthly program meetings for DSPs.
- Purchasing:
Purchases food, household and office supplies; monitors credit card statements and invoices. - Individuals’ Served Funds Management:
Secures and safeguards individuals’ funds; updates ledgers and maintains receipts. - Billing and Utilization:
Reviews census, attendance and other documentation for billing accuracy. - Staffing:
Monitors staffing hours for budgetary compliance.
- Census Capture:
Captures daily census accurately. - Census Performance:
Understands census measurement and performance; escalates potential service disruptions. - Placements:
Supports location preparations for expected placements.
- Vehicles:
Inspects vehicles monthly; ensures safety and maintenance. - Living Environment:
Monitors housekeeping, lawn and yard care; maintains a safe environment. - Maintenance and Repair:
