We have an open position for a Social Worker who will be an essential member of each patient’s core care team. You will support and advocate for patients transitioning to dialysis, helping them understand their rights, responsibilities, and manage the physical, mental, emotional, and financial demands of End Stage Renal Disease.

- Meeting all state‑required regulations to practice in the dialysis setting.
- Master’s degree in Social Work (MSW), required with a specialization in clinical practice.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, Outlook, and functional proficiency with DaVita‑specific applications within 60 days.
