General Summary

The Substance Use Disorder (SUD) Navigator functions as key support and coordination resource for individuals affected by substance use disorders, helping them access timely, appropriate, and continuous care. In this capacity the incumbent is responsible for providing a variety of substance use treatment outpatient services to members with mental illness and addiction disorders. The SUD Navigator functions include screening and identifying individuals with substance use needs, re‑engaging members, providing brief interventions and education, facilitating referrals to treatment, recovery, and harm‑reduction services.

JOB

RESPONSIBILITIES
  • Maintain confidentiality of records relating to member's treatment.
  • Assess members for substance use history, readiness to change, and level of care recommendations.
  • Collect information about members through interviews, observation or assessments.
  • Counsel members individually or in group sessions, to assist in overcoming dependencies, adjusting to life or making changes.
  • Guide members in the development of skills or strategies for dealing with their problems.
  • Develop and implement individualized recovery‑oriented treatment and crisis plans based on clinical experience and knowledge.
  • Collaborate with other staff members to coordinate member care and develop treatment plans.
  • Refer members to community resources or to specialists, as necessary.
  • Conduct community outreach to members, as well as visit members in detox centers, hospitals, and other facilities to complete recovery and crisis plans.
  • Act as member's advocate to coordinate required services or to resolve emergency problems in crisis situations.
  • Meet with families, probation officers, police, or other interested parties to exchange necessary information during the treatment process.
  • Attend interdisciplinary team meetings to collaborate with member's treatment team.
  • Maintain and submit detailed data required for management of opioid grant.
  • Perform other duties as assigned or necessary as they relate to the general nature of the position.
QUALIFICATIONS
  • Minimum

    Education Required:

    High School Diploma or Equivalent.
  • Minimum Experience

    Required:

    Behavioral health experience and / or substance abuse training preferred.
REGULATORY
  • Minimum 21 years of age.
  • Able to pass a minimum 5 panel drug test & submit to regular screening.
  • DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment).
  • Maintain a clear Central Registry Background Check.
  • CPR, First Aid, (must possess upon hire and maintain throughout employment).
  • Current, valid Arizona Driver's License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements.
