Job Responsibilities
- Ensures that medications are promptly, accurately, and safely dispensed and delivered.
- Supervises the work product of pharmacy technicians and other pharmacy staff.
To ensure that patients receive their medications safely, accurately, and timely.
Reports directly toPharmacist in Charge
Daily tasks- Review new referrals for completeness.
- Review medication profile and run interaction scan when appropriate.
- Review pertinent medical conditions.
- Ensure completeness of all patient profiles.
- Ensure all required information is obtained prior to servicing a patient (IV access, signed orders, following physician, nursing plan).
- Complete counseling, home safety evaluation, and initial assessment on every patient.
- Counsel patients.
- Review and complete all follow-up assessments and take appropriate action accordingly.
- Review and update care plans regularly.
- Review and track all lab results.
- Review nursing notes and take appropriate action accordingly.
- Manage Care Tend queues, daily tasks, and workflow to ensure all activities are completed accurately and on time.
- Complete all steps of production verification.
- Verify and mathematically check all ingredients entering cleanroom to be compounded.
- Program infusion pumps (final check must be a pharmacist).
- Ensure the cleanliness and organization of the pharmacy at all times.
- Communicate with doctors’ offices for verbal orders, order clarification, and therapy updates.
- Prepare and print all initial prescriptions and delivery tickets.
- Supervise cleanroom activities and sterile technique.
- Compound when necessary.
- Recommend therapies and offer solutions to clinics and patients when appropriate.
- Compose clinical prior authorization letters when appropriate.
- Complete all required training, competencies, and certificates on time.
- Complete applicable On-Track tasks on time.
- Uphold all pharmacy policies, procedures, competencies, and activities of all pharmacy personnel.
- Act as a resource for clinics, nurses, and patients.
- Complete verification of anaphylactic kits and supply kits.
- Participate in on call and holidays ritation, phsrmacy activities, and other duties as assigned.
