JOB SUMMARY:
The position is responsible for coordinating clinical activities and operations taking place in the assigned villas. Duties include managing care of all residents, providing oversight of the staff regarding activities in the community, communication with the care line and families about the needs of residents, maintaining compliance with all regulatory standards and communicating with executive leadership (Executive Director and/or Director of Health and Wellness) on all appropriate matters.
Provides direct supervision of caregiving within the community. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community.
Looking for 2 years experience as Resident Care Coordinator, with proven leadership.
Arizona Caregiver Certification Required
PRIMARY RESPONSIBILITIES:- Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions which assist others beyond job responsibilities, and has a positive impact on co-workers, residents, and visitors. Has working knowledge of the organization.
- Provide supervisory support by taking attendance at the beginning of every shift and ensure all houses are staffed accordingly for cares and med pass. Work with and/or update the scheduler to adjust employees’ hours and location assignments to reconcile scheduled employee shifts to the actual worked hours and locations.
- Verifies all associate hours on Deputy that coincide with RCCs specified Villas. Assists in completing care staff schedules to assure proper coverage to meet resident needs and according to skillset. Weekly meeting with scheduler and DHW. Assist with interview process for Certified and Assistant Caregivers as needed.
- Communicates with residents, family, and staff on an as needed basis to meet resident needs. Participates in care conferences with residents and family members to discuss resident needs and appropriate service delivery in association with service plan updates.
- Responsible for training and supervising all care staff and activity staff in specified villas. This includes managing performance, coaching to improve skills, directing staff to provide activities that correspond to service plans and appropriate documentation and compliance. Assures that all staff maintain appropriate annual training to keep up with state requirements in conjunction with human resources.
- Responsible for managing the villa environment cleanliness, upkeep and state survey compliance by completing daily scheduled rounds and addressing concerns and following up on resolution.
- Ensure maintenance work orders are communicated and follow through with any work orders that are not completed timely.
- Assists the DHW in medication management, including medication refills and cycle fill, administration of medications to residents, ordering medication from pharmacy and maintaining records as dictated in company policies and procedures manual. Ensures that medications are properly stored, medication carts/cupboards are cleaned, and medication room is organized according to standards.
- Observes residents' health statuses, takes actions to address concerns within scope of practice, and reports all significant change of condition, resident hospitalizations, reactions to medications and treatment, or significant incidences to care line in a timely manner. Reviews capabilities of resident who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation).
- In charge of pre-move in paperwork review (including diet, preferences, initial checklists, and vitals in PCC) and 72-hour move in protocol. Assures that all required documentation including consents, behavioral and regular service plans are completed and updates caregivers in home of resident prior to move in. The RCC must be present upon resident move-in to create a smooth transition for the caregivers and resident.
- Performs ongoing observations of resident physical and psycho-social needs and coordinates with care line to assure quality proactive care as allowed by state regulation.
- Communicates with families regarding residents' personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments, and supplies. Ensure residents are attending appointments as scheduled by transportation.
- Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families.
- Performs daily, weekly, and monthly safety and infection control checks of the home and grounds for resident health and safety purposes. Communicates concerns with DHW and Executive Director/GM.
- Participates in the maintenance of resident records including review of documentation performed by resident assistants, monthly MAR reviews, setting up new resident and thinning records appropriately.
