Physical Therapist Assistant – $5,000 Sign‑On Bonus

• Pay – $28.00‑$48.00 per hour

• Provide physical therapy services and interventions under the direction of a physical therapist and according to the physician‑approved plan of care.
• Observe, record, and report the patient’s response to treatment, including any change in condition.
• Promote wellness and prevention, providing resources for future needs.
• Document supervisory activities in the patient’s medical record.
• Monitor and document the patient’s response to physical therapy intervention.
• Participate in patient‑care conferences, supervision services, chart audit, quality improvement activities (e.g., OBQI, OBQM), and peer‑review activities.
• Maintain confidentiality of information related to the therapist‑client relationship.
• Recommend follow‑up care, coordinating with the agency that admitted the patient and other care providers to assist with community referrals.
• Perform other duties as assigned.

• Current license to practice as a PTA in the state of assignment.
• Graduate of an accredited PTA educational program as set by the CAPTE.
• One year of related PTA experience in home‑health care, or if less, approval from regional clinical leaders and appropriate mentoring or residency participation.
• Current CPR certification.
