* Job Description:
** - Are you ready to make a significant impact in the field of emergency nursing and mental health? - We are seeking an enthusiastic Nurse Case Manager for the Emergency Department to join a vibrant and diverse healthcare team!
** Responsibilities:** - Collaborate with patients, families, and a multidisciplinary team to ensure seamless care across the healthcare continuum.
- Assess patient needs and determine the most appropriate level of care, from outpatient to inpatient settings.
- Engage in effective discharge planning and facilitate referrals to various levels of care.
- Utilize your knowledge of Inter Qual Criteria to evaluate patient status and medical necessity for admissions.
- Work closely with advanced practice clinicians and physicians to propose and implement safe discharge plans.
- Coordinate care for various patient populations, including those facing mental health challenges and chronic pain.
- Maintain professional relationships with community resources and payers to enhance patient outcomes.

* Qualifications:
**
- Education:
Associate’s degree in Nursing.
- Licensure:
Valid, unrestricted Professional Nursing License in any U.S. state, Puerto Rico, or a territory.
- Certification:
Current Basic Life Support (BLS) certification by the American Heart Association;
Case Manager Certification (CCM or ACM) within 2 years of hire.
- Experience:
- Minimum of 2 years of care/case management experience in an Emergency Department setting.
- 2 years of patient care experience (Hospital, Home health, Skilled Nursing).
- Proficiency in Electronic Medical Records, Outlook, Microsoft Office Word, and Excel.
- 1 year of travel experience is a must.
** Shift Information:**
- Work schedule:
Sunday - Saturday, 10 shifts, variable 7 am - 5:30 pm with weekend rotation and call responsibilities.
