Advanced Practice Provider – Primary Care Outpatient

Responsibilities

• Clinical Care: Provide assessment, triage, and therapeutic management for patients across the disease process. Work with supervising physicians within state laws and hospital policies to deliver autonomous, patient-centered care and participate in operative assistance as allowable.
• Patient & Psychosocial Support: Educate patients and families on care plans, medication management, and post-discharge instructions; offer psychosocial support as part of a multidisciplinary team.
• Information Management: Maintain accurate documentation per hospital and regulatory standards, safeguarding patient data across all media.
• Quality Management: Uphold high ethical standards, treat patients with compassion, and ensure compliance with regulatory requirements and institutional protocols.
• Professional Development: Engage in competency-enhancing projects, serve as a clinical resource for new staff, mentor students, and contribute to research, quality improvement, and protocol development.

Qualifications

• Education: Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Experience: Prefer clinical experience as NP/PA in a related specialty. All applicants must be appointed to the Affiliated Medical Staff of a YNHHS hospital and credentialed via the Medical Staff process.
• Licensure: Active PA certification (NCCPA), Connecticut PA license, and ACLS/BLS certification. Additional licenses (DEA, PALS, NRP) required per specialty. For RN/NP applicants:
  Connecticut RN license, Connecticut NP license, board specialty certification, and ACLS/BLS. Other required certifications may be added as needed.
