Social Services Director (LSW Required)

Troy Rehabilitation and Healthcare Center is hiring a full-time Social Services Director to lead our Social Services department and support residents and families throughout their stay.

• Oversee daily operations of the Social Services department

• Complete psychosocial assessments and care plan documentation

• Provide resident and family support, advocacy, and resource coordination

• Lead discharge planning and coordinate transitions of care

• Participate in care conferences and interdisciplinary team meetings

• Ensure compliance with state and federal long-term care regulations

• LSW required

• Prior experience in a skilled nursing/long-term care or healthcare setting required

• Strong communication, documentation, and organizational skills
