Job Description:
- Provider's choice, we are open to any and all availability - (full-time or part-time options)
- 7:00 am-3:00 pm OR 8:00 am-4:00 pm OR block design available as well - 8, 10, or 12 hour shifts
- Call Rotation: 1:6 with post call day off, optional but preferred.
- Surgical Cases are primarily bread and butter including OB, Bariatric, Vascular, and Urology. No hearts, trauma, or Neuro cases.
- EMR:
Epic - Not accepting Locums candidates
- Salary Range: ####,000-####,000
- Starting Bonus

- CME time & expense allowance

ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.
- Board Certified (NBCRNA)
- REQUIRED - NY License (or willing to obtain)
- REQUIRED - Note if provider is interested in FT or PT employment (add to "Additional Submission Details" field)
