Registered Nurse-Patient Navigator (Full Time) Days - Farmington Hills

Location:

Farmington Hills, Michigan (may travel to other locations in the area if needed)

Monday-Friday 8:00 AM - 4:30 PM (no weekends, holidays, or on-call)

Full Time

• Serve as a patient navigator throughout the continuum of care.
• Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
• Coordinate efforts for treatment and promote quality improvement and educational efforts.
• Compile data, track outcomes, and make recommendations for process improvement.
• Provide referrals/support for follow-up care for patients who are uninsured or underinsured.

• Diploma from an accredited school/college of nursing OR professional licensure at time of hire.
• Minimum of 2 years of Registered Nurse Med-surg experience.

• Experience with patient care plans.
• Comfortable with Microsoft office suite applications (Excel, Word, Calendar, Outlook, Teams).
• Comfortable with interacting with patients both face-to-face and telephonic.
• Comfortable with communicating with other disciplines (physicians, social workers, office staff, community health).
• Comfortable with documenting in EHR and navigating EHRs.
• Strong communication skills.

• Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date.
• BLS Provider credentialed from the American Heart Association (AHA) or American Red Cross preferred.
