Position: CEAS Case Manager (F/T)

CEAS (Coordinated Entry Assessment System) Case Manager (Full-time)

Having served the Trenton area for more than 47 years, Anchor House has been providing services to youth and young adults who have run away and those that are experiencing or at risk of homelessness, abuse, or neglect. We are looking to add to our dynamic team!

We are currently seeking a CEAS Case Manager
, for our Trenton based programs, to join our team. In this role, you’ll directly impact the lives of young adults who are homeless, are runaways or living on the streets by providing crucial outreach, crisis intervention, and support services to help them transition to safety and stability. By building trusting relationships, offering guidance, and connecting youth with resources, you’ll empower them to leave the streets and access opportunities for a brighter future.

This position is Monday-Friday from 9 a.m. to 5 p.m.

Salary Range: $50,000-$53,000

What You’ll Do
  • Adhere to all Anchor House Policy and Procedures Manual;
  • Provide gateway services to youth/young adults that are homeless, living on the streets or runaways;
  • Provide outreach and education services to youth/young adults that are homeless, living on the streets or runaways aimed at building relationships between the population and program staff to encourage youth to leave the streets;
  • Provide coordinated case management for youth accepting services;
  • Provide crisis intervention to youth who are at risk for sexual abuse and exploitation;
  • Provide street youth access to local emergency shelter space and transitional living;
  • Provide access to individual assessment and counseling with youth experiencing substance abuse issues;
  • Provide referrals;
  • Make educational presentations to youth, parents and community members on the dangers of street life and services provided by Anchor House and other agencies;
  • Provide life skills training and activities;
  • Provide a wide range of assistance and support to youth;
  • Provide transportation to youth in the program;
  • Teach advocacy skills;
  • Utilize HMIS for data entry and reporting;
  • Maintain accurate statistics and client records, completing them on a timely basis using spreadsheet/database programs and paper documentation.
  • Provide aftercare follow-up services, including case management and service linkages to community agencies;
  • Work closely with the Trenton/Mercer Continuum of Care (CoC) team and its partners;
  • Attend and participate in all required meetings;
  • Recruit youth through outreach to participate in activities;
  • Build community relations and partnerships to enhance group activities;
  • Be trauma-informed when working with all youth;
  • Maintain accurate records;
  • Participate in ongoing program evaluation and planning and best practice implementation;
  • Participate in all agency staff meetings and trainings as required;
  • Be flexible in providing service hours that are conducive to working with youth;
  • All other responsibilities that may be necessary to fulfill the duties associated with the position or assigned by the supervising manager.
What You Bring
  • Minimum Bachelor’s Degree in social work, psychology or other human services related field;
  • Minimum of two years’ experience working with children and youth;
  • Must possess a valid driver’s license and provide an abstract for review;
  • Must be able to pass all criminal background checks including CARI and CHRI;
  • Effective communication skills including written and verbal;
  • Ability to work well in a team setting;
  • Ability to work independently and under pressure
