Social Worker, Healthcare Administration

Overview

Join our team at Avalon Rehabilitation and Healthcare Center as a licensed Social Worker.

Rate Range: $25/hr.

-$50/hr. (All Inclusive based on experience)

Must have SNF/LTC experience

• Evaluate patients (existing, new, and prospective) based on their needs and limitations.
• Address patient concerns and goals while maintaining constant communication with the patient and the case manager.
• Build rapport with patients and their families
• Coordinate care with PT departments
• Develop care plans for patients

Qualifications of Social Worker: MUST BE Licensed

• Bachelor’s degree required. Master’s degree is strongly preferred.
• Experience as a social worker in a healthcare-related setting. Experience with in a nursing home or rehabilitation center is strongly preferred.
• Knowledge of the rules, regulations, and guidelines that govern nursing care facilities.
• Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.
