The listed minimum pay reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

Position Overview

Essential Functions

Min Salary: $37.13

  • Documents clinical record. Assesses comprehensively the patient needs including barriers to timely discharge, clinical interventions, support, and appropriate community resources according to policy. Provides individual and group therapy as appropriate.
  • Uses culturally competent skills to assess individual patient needs and refers to appropriate professional staff when necessary. Identifies barriers and plans for intervention to overcome or lessen barriers to achieve outcome as evidenced by treatment plan.
  • Documents clinical record and appropriate interventions including assessment, plans, and referrals in the discharge planning process. Collaborates, educates, and assists health care team to identify social and financial barriers that delay appropriate transitions through multidisciplinary rounds and medical record documentation.
  • Collects and revises information on community resources to ensure complete and up to date awareness by patients.
  • Documents accurately clinical record of all referrals and clinical interventions related to abuse, neglect, domestic violence, alcohol, and substance abuse and crisis intervention.
  • Performs educational sessions for healthcare team related to abuse and neglect issues and reporting of same. Documents staff interventions in clinical record.
  • Exercises discretion and judgement when counseling staff and patient and family on conflict management in health care setting and monitors reaction and need for ongoing intervention.
  • Advocates for appropriate services for all patients with payer and providers of post-acute care service, community service, and other services in order to optimize patient outcome.
  • Attends and participates in regular departmental performance programs, staff meetings, and hospital wide committees to analyze and respond to issues significant to social work services and to identify opportunities for improvement.
  • Assists in data collection and information gathering on patient care issues, program, and systems analysis in order to comply with plan, do, check, act performance improvement standards of DNV.
  • Provides clinical supervision to CSW and LSW staff in the minimum to meet Department of Health and NJ Board of Social Work Examiners requirements.
  • Continually reviews regulatory requirements for social work and discharge planning services to assure appropriate and mandatory services are provided.
  • Obtains pre-certifications as needed.


Minimum Requirements

  • Education :

    Master's degree in social work.
  • Experience:

    Five years experience in a behavioral health setting.
  • Other Credentials:
    Social Worker-Lic/Clinical
  • Knowledge and

    Skills:

    Possesses excellent organizational, interpersonal, conflict resolution, and verbal and written communication skills. Ability to effectively manage multiple projects simultaneously and ability to respond quickly in a fast paced environment.
