Schedule
The on-call Hospice nurse will work a 7-day on, 7-day off schedule, from 3:00 pm to 8:00 am, Monday through Friday, and all weekend.
OverviewWe are seeking a compassionate and skilled Hospice Nurse to join our dedicated team. The ideal candidate will possess a strong background in nursing with experience in hospice and palliative care. This role involves providing high-quality patient care to individuals with terminal illnesses, ensuring comfort and dignity during their final stages of life. The Hospice Nurse will collaborate with interdisciplinary teams to develop and implement care plans that address the physical, emotional, and spiritual needs of patients and their families.
Responsibilities- Conduct comprehensive patient assessments to evaluate physical, emotional, and psychosocial needs.
- Develop individualized care plans in collaboration with the healthcare team, focusing on pain management and symptom control.
- Administer medications, IV infusions, and treatments as prescribed, ensuring adherence to safety protocols.
- Provide education and support to patients and families regarding end-of-life care options and resources.
- Monitor patient conditions regularly, documenting changes in medical records in compliance with HIPAA regulations.
- Facilitate communication between patients, families, and healthcare providers to ensure continuity of care.
- Participate in case management activities, including discharge planning and coordination of services.
- Utilize EMR systems for accurate medical documentation and patient data management.
- Engage in ongoing professional development to stay current with best practices in hospice care.
- Current nursing license (RN) in the state of practice.
- Experience in hospice & palliative medicine preferred; familiarity with long-term care settings is a plus.
- Proficiency in medical terminology, patient assessment, medication administration, and basic life support.
- Knowledge of ICD-10 coding, NCQA standards, and relevant healthcare regulations.
- Critical care experience is highly desirable; familiarity with trauma medicine or emergency medicine is an asset.
- Ability to work effectively with individuals who have developmental disabilities or chronic illnesses.
- Strong communication skills for effective interaction with patients, families, and multidisciplinary teams.
- Familiarity with various EMR/EHR systems such as Epic or Cerner is advantageous.
