Lake City / Cadillac or Traverse City, MI (options). Parent agency located at Charlevoix.

• Provide emotional support to help patients live their final days with dignity and comfort.
• Conduct supervisory visits with assigned team members for clinical observation and performance review.
• Participate in interdisciplinary group meetings, providing feedback on patient/family care plans.
• Assist in the development, review, and revision of the patient/family plan of care.
• Prepare clinical and progress notes and participate in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current processes and technology within the organization.

• Registered Nurse with a current Michigan license in good standing.
• Minimum 1 year of hospice experience.
• Empathetic approach to patients and caregivers.
• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement individualized plan of care and revise as needed.
• Compliance with Advisa Care Hospice Policies and Procedures, Medicare compliance.
• Consulting with and educating patient/family on disease process, self‑care techniques, end‑of‑life care, and ethical issues.
• Supervision of paraprofessionals providing services to patients.
• Participate in field visits daily, weekly office meetings as indicated.
• Adapt to patient status and needs on a full‑time on‑call basis.
