Job Overview
A Transitional Care Nurse Practitioner coordinates and manages patient care during transitions between care settings, such as from hospital to home or post-acute facilities. - Master’s Degree (Required)
- Certified Registered Nurse Practitioner (Required)
- Primary Care or Transitional Care NP Experience Preferred
- Strong Verbal Communications Skills, Strong Written Communications Skills, Excellent Interpersonal Skills, Basic Computer Skills, Excellent Organizational Skills, Strong Customer Services Skills, Proven Analytical Skills
- Pay Range: $53.95-$91.81
