Transitional Care Clinic Nurse Practitioner

Job Overview

A Transitional Care Nurse Practitioner coordinates and manages patient care during transitions between care settings, such as from hospital to home or post-acute facilities. This role focuses on reducing readmissions by providing comprehensive assessments, medication reconciliation, patient and caregiver education, and timely follow-up care. The nurse practitioner collaborates closely with physicians, care management, and community providers to ensure continuity of care.

Strong clinical judgment, communication skills, and a patient-centered approach are essential to improving outcomes and supporting high-quality transitions of care.

• Master’s Degree (Required)

• Certified Registered Nurse Practitioner (Required)

• Primary Care or Transitional Care NP Experience Preferred
• Strong Verbal Communications Skills, Strong Written Communications Skills, Excellent Interpersonal Skills, Basic Computer Skills, Excellent Organizational Skills, Strong Customer Services Skills, Proven Analytical Skills

• Pay Range: $53.95-$91.81
