• Conduct comprehensive in‑home assessments & reassessments
• Create and update care plans
• Provide skilled nursing interventions & medication administration
• Monitor and document client health changes
• Communicate with clients, caregivers, and healthcare teams
• Coordinate referrals to healthcare or community resources
• Maintain accurate documentation per state & agency regulations
• Participate in caregiver training and in‑service programs

• Current Maryland RN license (or multi‑state compact license)
• CPR/First Aid certification ❤️
• Home Health, Hospice, or Geriatric Nursing experience preferred
• Strong assessment & clinical decision‑making skills
• Reliable transportation for home visits
• Willingness to work occasional evenings/weekends
• Knowledge of infection control & patient safety procedures
