We undertake holistic assessment within the home environment, identifying needs and priorities with patients and their carers to improve wellbeing, maintain independence with an overarching aim to prevent unplanned admissions to hospital.
The Team currently comprises two Registered Nurses (RNs), an Occupational Therapist (OT), two Assistant Practitioners (AP), and a Social Prescribing Link Worker. The CCT operates Monday to Friday, 08:30 to 16:30 excluding Bank Holidays
What our Patients say about us:
The CCT are a fantastic Team! The Nurse who visited was very knowledgeable and kind, she helped me make sense of my new medication. The OT has arranged lots of equipment and some grab rails to keep me safe and independent in my own home. I was also seen by a lady who helped me apply for Attendance Allowance and a Lifeline, the extra money will really help my quality of life, and the Lifeline will reassure my family that I am safe.
