Towcester Medical Centre is seeking an experienced and motivated Advanced Clinical Practitioner to join our friendly and supportive multidisciplinary team.

We are looking for an Independent Prescriber from a Nursing or Paramedic background with significant experience in primary care or a relevant clinical setting. The successful candidate will provide high-quality, patient-centred care within GP practice, including initial history taking, comprehensive clinical assessment, diagnosis, treatment, prescribing, and ongoing evaluation of care.

Main duties of the job

Attend to patients presenting with a wide range of acute and chronic medical conditions, providing expert clinical assessment and advice.

Undertake comprehensive history taking, clinical examination, diagnosis, treatment, and ongoing monitoring of patients within the Primary Care setting.

Deliver high-quality patient care for the management of long-term and chronic conditions in line with local and national guidelines.

Maintain accurate, timely, and contemporaneous clinical records using appropriate SNOMED coding.

Support the wider clinical team in the management of clinical and medical emergencies.

Practice at all times in accordance with the relevant professional code of conduct and scope of practice.

Prescribe independently within areas of clinical competence and in line with current legislation and best practice.

Initiate, evaluate, and modify a range of clinical interventions, including prescribing medications, recommending therapies, providing lifestyle advice, and coordinating ongoing care.

Towcester Medical Centre is a forward thinking training practice situated in a pleasant semi rural market town with good transport connections.

We are high achieving and innovative in our approach and very keen to develop our staff to their full potential.

We have just over 11,700 patients, 5 GP partners, 3 salaried GPs, an experienced nursing team of 3 which includes 2 HCAs and strong administrative support. This gives us a strong basis to provide excellent patient care.

Our team are very friendly, and this is often commented on by patients, new starters and visitors to the Practice.

Job responsibilities

Practice autonomously as an Advanced Clinical Practitioner within Primary Care, working within agreed clinical scope of practice and local protocols.

Manage patients presenting with a range of acute and chronic medical conditions, providing safe, evidence-based clinical assessment and advice.

Undertake comprehensive history taking, clinical assessment, examination, diagnosis, and treatment planning for patients presenting with a variety of conditions.

Act as an Independent Prescriber (V300), prescribing safely and appropriately within areas of clinical competence.

Assess and manage all patients presenting with minor illness, in line with practice policy and clinical guidelines.

Work collaboratively with GPs and the wider multidisciplinary team to ensure coordinated, high-quality patient care.

Take an active role in clinical supervision, mentorship, and reflective practice to support ongoing professional development.

Deliver chronic disease management clinics as required, in line with national and local guidelines.

Refer patients to secondary care services appropriately, following agreed referral pathways.

Maintain accurate, timely, and comprehensive clinical records, including the use of SNOMED coding.

Person Specification Experience
  • Experience of working in Primary Care
  • Experience of prescribing
  • Experience of providing patient care autonomously.
  • Ability to work within own scope of practice and understand when to refer to GPs
  • Knowledge of Optum (EMIS) Clinical System
  • Knowledge of clinical governance
  • Understand the requirements of PGDs and associated policy
Qualifications
  • Registered with NMC or HCPC.
  • Independent prescriber V300
  • Educated to Masters degree as detailed within the HEE document:
