Community Nurse Team Leader

We are looking for an enthusiastic and compassionate Community Nurse Team Leader to lead our Totnes Community Nursing team. Main duties of the job
  • To lead and deliver high quality care to the housebound patients of Totnes.
  • To ensure the effective co‑ordination and provision of the district nursing service in the absence of the District Nurse Lead.
  • To actively participate in the planning and discharge of complex patients from secondary to primary care.
  • Work as part of multi‑disciplinary team and in partnership with primary care and secondary care.
  • Can provide motivational and negotiation skills in areas where consent/ co‑operation is required or there may be barriers to understanding. To include undertaking best interest decisions.
  • To ensure timely and effective communication with the team manager and other team members on professional matters.
  • To liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problem.
  • Strong communication is essential, as we work closely with a wider multi‑disciplinary team, particularly our Intermediate Care Team, to support seamless, integrated care.
  • Ability to provide motivational and negotiation skills in areas where consent/ co‑operation is required or there may be barriers to understanding.

To monitor and evaluate care programmes for patients and carers, promoting effective co‑ordination and continuity of patient care.

To communicate formally and informally with the DN team leader to ensure best practice and consistency with the community and intermediate care service in TSDFT.

To be responsible for the performance of risk assessments for patients, staff and carers, in various working environments and act on the results.

To utilise teaching, assessments and mentoring skills with students and team members. To provide an effective learning environment.

To identify opportunities for professional development of team members through the development review and appraisal process.

An ability to provide and receive complex, sensitive and contentious information: using empathy and reassurance skills.

To liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problems.

To undertake Specialist Nursing and Continuing Care assessments (with appropriate training).

Person Specification Qualifications and Training
  • Qualified Registered Nurse Level 1
  • BSc (Honours) in Community Health Nursing (District) or equivalent or working towards
  • V300 Independent/Supplementary Prescriber
Knowledge and Experience
  • Proven experience in leadership skills
  • Able to work as an autonomous practitioner
  • Proven relevant I.T. knowledge and experience
  • Evidence of on‑going training and development
