Job Summary:

We are seeking a dedicated and compassionate Licensed Practical/Vocational Nurse (LPN) to join our team at Torrington Center for Nursing and Rehabilitation. As an LPN, you will play a vital role in providing high‑quality patient care to our residents, while maintaining a warm and welcoming environment.

• Provide direct patient care to residents, including administering medications, wound care, and performing vital signs
• Develop and implement individualized care plans, in collaboration with the interdisciplinary team
• Monitor and report changes in patients’ conditions to the Nursing Supervisor
• Maintain accurate and detailed records of patient care and treatment
• Participate in patient education and discharge planning
• Collaborate with the interdisciplinary team to ensure comprehensive and individualized care

• Current LPN licensure in the State of Connecticut
• 0 years of experience required; willingness to learn and grow with our team
• Ability to lift, transfer, and reposition patients as needed
• Strong communication and interpersonal skills
• Ability to maintain confidentiality and adhere to HIPAA guidelines
