We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at the Torrington Center for Nursing and Rehabilitation, providing exceptional care to our short-term rehabilitation and nursing care patients in Torrington, Connecticut. As an LPN, you will be responsible for delivering high-quality patient care under the supervision of our experienced nursing staff.

• Provide direct patient care as assigned, including vital sign checks, medication administration, and wound care
• Assist patients with activities of daily living (ADLs), such as bathing, dressing, and grooming
• Monitor patient's conditions and report any changes or concerns to the registered nurse or physician
• Collaborate with the interdisciplinary team to develop and implement patient care plans
• Maintain accurate and detailed records of patient care and treatment
• Participate in quality improvement initiatives to enhance patient outcomes

• Current LPN license in the state of Connecticut
• High school diploma or equivalent required
• 0 years of experience as an LPN or related healthcare experience
