16 to 32 hour position. Any combination of days Friday through Monday but must include Saturday and Sunday. 8 hours per day. Benefits apply if 24hrs or more.

• Facilitate communication between the patient and the interdisciplinary team.

• Utilize state-of-the-art technology to document skilled services provided.

• Licensed Practical Nurse with valid CT LPN license.
• One year of LPN experience;
  Homecare experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.
