Location Detail: 65 Commercial Blvd, Torrington (10413)

Shift Detail: Monday thru Friday 8am to 4:30pm with 1 major and 1 minor holiday per year and 1 weekend per month.

Responsibilities
  • Manage and coordinate the clinical intake process for Home Health and Hospice services across multiple branches.
  • Ensure timely, accurate, and compliant intake procedures using the Homecare Homebase (HCHB) system.
  • Serve as a key liaison between referral sources, clinical teams, and operational leadership.
  • Perform all clinical aspects of the intake process, including communication with patients and families, admitting clinicians, clinical data entry accuracy, timeliness, face‑to‑face encounter accuracy, and PCP/MD verification.
  • Ensure patients are admitted timely and in accordance with the intake policy.
  • Provide recommendations for process and policy improvements.
  • Ensure compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
  • Stay thoroughly informed about the details of clinical quality metrics and report key data to Home Care Leadership and partners.
  • Establish and maintain positive working relationships with current and potential referral sources and associated clinical leadership teams.
  • Coordinate patient education, care planning, and coordination with multiple providers and case managers within established payer and financial guidelines.
  • Strengthen and monitor community and customer perceptions of Hartford Healthcare at Home as a high‑quality service provider, collecting and reporting clinical referral data and trends.
Qualifications
  • Education:

    Licensed clinician (RN, LPN, or equivalent) required;
    Bachelor’s Degree Preferred (BSN, Healthcare Administration or related field).
  • Experience:

    1 year of administrative/clinical healthcare experience; 1–3 years experience preferably in Home Care.
  • Knowledge, Skills and Ability:
    Experience in referrals/intake in a home health or hospice environment; strong communication, negotiation, and public relations skills; autonomy, assertiveness, flexibility, and cooperation in performing job responsibilities.
