Job Description
Home Health RN - Adult/Geriatric

Schedule
- Shift start: 8:00 AM
- 8-hour shifts
- Weekend requirement:
Coverage approximately every 4 weeks; contract specifies 2 weekend shifts
- On-call:
Yes

Assignment Details
- Daily caseload: 5-6 patients
- Patient population:
Adults/Geriatrics

Requirements
- Active CA RN license required at time of submission
- Minimum 2 years home health experience with high-volume caseloads
- Recent OASIS experience required
- Previous home health travel experience required
- BLS certification required
- Flu vaccine required

Roles & Responsibilities
- Provide comprehensive nursing assessments for patients in their homes.
- Manage wound care including V.A.C. and PICC dressing changes.
- Conduct start-of-care workloads with expectation of approximately 3 starts in 12 hours.
- Perform IV therapy and medication administration.
- Ensure infection control practices are followed during patient care.
- Document patient care and progress in accordance with OASIS standards.

Compliance / Notes
- Background checks performed; DUIs may be considered depending on recency.
- References: 2 references from within the past 1 year required; must be from a supervisor or higher.
- Time-off request not allowed within the first 2 weeks; total RTO should not exceed 10 days. RTO must be provided upon submission.
