Description: Are you a skilled Registered Nurse with a passion for delivering high-quality care in the comfort of patients' homes? We're seeking a dedicated Home Health RN to join our team in the Santa Monica/West LA and surrounding areas. As a Home Health RN, you'll have the opportunity to make a meaningful impact on patients' lives while enjoying the flexibility and autonomy of home health care.
Responsibilities:
- Provide compassionate and comprehensive nursing care to patients in their homes
- Perform patient assessments, develop individualized care plans, and implement appropriate interventions
- Administer medications, monitor vital signs, and educate patients and their families on disease management and self-care techniques
- Coordinate care with interdisciplinary team members and community resources to ensure continuity of care
- Document all patient interactions and interventions accurately and timely
- Active Registered Nurse license in the state of California
- Minimum 1 year of skilled Home Health experience required
- BCLS certification
- Ability to work independently and manage time effectively
- Strong communication and interpersonal skills
- Must have own transportation
- Oasis experience preferred, but not required
- Willingness to travel within a 50-mile radius, with occasional longer drives

- Mileage reimbursement at IRS standard rates
