Position:
Life Skills Coach 1 - Site-based

Location:

Carson Site (Torrance)

Hours:

Part-Time - Monday-Friday - 9:00 am to 2:30 pm

Hourly Pay Rate: $20

* We provide CPR/First Aid certification for Adult Day Services*

Responsibilities
  • Assist individuals with assessments, planning, and the evaluation process. Support individuals in meetings with their support team and advocate for changes to the Service Plan as necessary.
  • Develop and implement an Individual Service Plan (ISP) aligned with the individual’s personal goals. Maintain documentation including ISP documents, quarterly and annual evaluations, and case notes.
  • Assist individuals in selecting person-centered activities and life skills experiences in line with ISP and IPP objectives.
  • Collaborate with direct support staff to plan and deliver activities that meet service objectives.
  • Supervise individuals in the service and provide personal care support as required, including nutrition and personal hygiene, and helping individuals look their best.
  • Support individuals in communication and educate them on tools to help them express needs and choices.
  • Safely use technology, adaptive equipment, and assistive devices; train individuals in their use as directed.
  • Follow procedures, standards, guidelines, and regulations to ensure health and safety and quality of services; attend trainings and required meetings.
  • Perform other duties as assigned.
Qualifications
  • Active listening, patience, and empathy in communication.

  • Ability to build positive relationships with associates, stakeholders, individuals, families, caregivers, and the public.
  • Good judgment, discretion, and handling of confidential matters.
  • Ability to interpret and follow policies, procedures, and regulations.
  • Basic computer proficiency and ability to input data in service applications.
  • Ability to provide personal care assistance, including assisting with transfers with or without weight-bearing capacity.
  • Conflict resolution skills and the ability to remain calm in stressful environments.
  • Physical ability to lift up to 50 pounds repetitively, walk, stoop, stand, kneel for extended periods, push wheelchairs on various terrains, and tolerate outdoor conditions. Ability to move and rearrange furniture as needed.
  • Ability to travel locally with individuals to participate in activities related to the service.
  • Ability to obtain and maintain a criminal record/fingerprint clearance per program requirements.
  • Ability to provide proof of required vaccinations or positive titer showing immunity.
  • Ability to pass post-offer medical examination and tuberculosis test; pass drug testing as required by ESSC.
Education & Experience
  • High School diploma.
  • Valid CPR and First Aid certifications.
  • Two to four (2-4) years of related experience preferred, or a combination of education, experience, and training to perform the job successfully.
