Discharge Lounge Sister/Charge Nurse

We’re looking for a highly motivated nurse to form part of the Discharge Lounge Service within the Trustwide Site Operations. The position works a 34.5‑hour week.
Responsibilities- Support the Clinical and Operational leads to manage and lead the Discharge Lounge.
- Coordinate the unit to maintain flow and patient journey, identify patients, transfer them around the hospital, complete handover processes and ensure safe discharges.
- Participate in assessment of care needs, development, implementation and evaluation of care plans, promoting high standards and patient dignity.
- Serve as a skilled member of the multi‑disciplinary team, promoting good inter‑departmental relationships.
- Act as a clinical adviser on nursing issues to maintain high standards of care using evidence‑based practice.
- Participate in direct care and facilitate sound discharge planning.
- Ensure high standards of care in relation to agreed standards, policies and procedures of the Trust.
- Participate in teaching and facilitate a learning environment, supporting clinical staff through appraisal and personal development planning.
- Effectively manage the discharge lounge, assisting the coordinator and Operational Manager with unit resources such as staffing, leave and sick reviews.
- Ensure adequate skill mix and proactive transfers and discharges, guaranteeing safety and earliest possible discharge.
- Assist in evaluating new nursing procedures, research and initiatives for incorporation into practice.
- Act as a councillor and advisor to junior staff and learners on nursing issues.
- Conduct appraisals and personal development planning for clinical staff.
- Participate in the MDT and ensure necessary discharge documentation is completed timely.
- Support and oversee escalation ward when operational.
- Registered Nurse with requisite clinical experience in acute adult inpatient care.
- Excellent working knowledge of discharge planning and patient flow in acute services.
- Strong leadership, communication and inter‑departmental relationship skills.
- Knowledge and application of evidence‑based practice.
