Mental Health Case Worker – Permanent Full-Time (City of Toronto)

The Mental Health Case Worker is responsible for providing direct hands‑on support to clients in their home using a harm‑reduction, client‑centered, anti‑oppressive, and trauma‑informed approach. The case worker will carry a case load of clients experiencing a range of challenges such as risk of homelessness, living in poverty, mental health issues, substance use, hoarding behaviours, trauma, social isolation, immigration challenges, unemployment/underemployment, and/or a history with the criminal justice system.

The position focuses on helping clients achieve housing stability, enhance their well‑being, and build the skills necessary to live independently in the community.

Key Areas Of Accountability:

  • Provide long‑term clinical case coordination services to a complex population of clients experiencing mental health, addictions, disabilities, and/or hoarding behaviours in collaboration with the client’s care team.
  • Conduct assessment of home support needs among program participants and collaborate with relevant supports to execute specific care plans.
  • Implement and execute service plan steps created by or in collaboration with the supervisor and clients.
  • Assist clients to achieve housing stabilization through problem‑solving associated with high‑risk indicators endangering clients’ mental and physical health and work toward creating a healthy and safe environment.

Key Skills, Experience And Behaviors Required For This Position:

  • Degree or diploma in a health‑related field (e.g., Social Work, Psychology, Nursing, Occupational Therapy, Addictions Counselling) or equivalent combination of education and experience.
  • At least two year’s clinical experience working in the community and providing case coordination services to vulnerable clients who have experienced homelessness, poverty, mental health issues, substance use, hoarding behaviours, trauma, social isolation, immigration challenges, unemployment/underemployment, and/or a history with the criminal justice system.
  • Utilizes an evidence base in clinical practice and possesses excellent consultation and teaching skills.
  • Proven ability to work from a client‑centered and harm‑reduction approach and has excellent problem‑solving, conflict resolution skills, crisis intervention techniques, and uses a solution‑focused approach.
  • Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.
  • Strong knowledge of community social and health services and established contacts with other agencies (an asset).
  • Highly motivated self‑starter who takes initiative, is results‑oriented, and can prioritize and work well both independently and as part of a team.
  • Excellent interpersonal and communication skills.
  • Ability to multi‑task, stay on track with expected deliverables, and problem‑solve in a fast‑paced environment.
  • Strong computer skills including Microsoft Office and ability to learn VHA’s client management software system.
  • Flexibility to provide services outside of normal business hours based on client needs and be available to work occasional evenings and weekends and between the hours of 8 AM – 7 PM.
  • Must have a valid Ontario driver’s licence, access to a vehicle, and willingness to travel across the city of Toronto.
