Working as a collaborative member of the inter‑professional team, the Nurse Practitioner (NP) functions in an expanded role demonstrating a high level of autonomy and expert skill to provide direct and comprehensive care and services to patients across the cancer care program ticipate and lead quality improvement initiatives and make recommendations for change in consultation with the Program Director, Manager, and Physician Leader.
Provide training, coaching, and education related to evidence‑based Oncology practice and knowledge among team members.
The Nurse Practitioner leads the provision of comprehensive cancer care services in SHN’s Cancer Care and Hematology Clinics. This model of cancer care employs interprofessional comprehensive cancer care along the continuum from diagnosis to survivorship and palliative care. The Nurse Practitioner will use advanced practice skills to provide assessment, coordination, navigation, and provision of services while integrating best practice guidelines and current evidence‑based research into clinical practice for cancer care and hematology patients.
SuspicionOf Cancer Clinic (SoC)
Suspicion of Cancer (SoC) is defined by persistent (>4 weeks), progressive, or unexplained symptoms, with early identified investigation abnormalities, giving rise to a clinical concern for an underlying malignancy that does not yet meet criteria for a disease‑site‑specific pathway or specialty referral. Suspicion of cancer is informed by the clinician’s overall clinical assessment and judgment, including situations where clinical concern remains high despite non‑specific or indeterminate findings.
SoC clinics are an Ontario Health – Cancer Care Ontario project (BEACON) aiming to improve the diagnostic experience for patients and providers. SHN’s SoC clinic is Nurse Practitioner lead, ensuring timely diagnostic assessment, strong navigation and care coordination, while advancing equity and access to care.
With a growing demand for cancer care it is a priority for SHN to provide comprehensive cancer care services within the Scarborough community. As an expert Oncology Nurse Practitioner, you will provide outreach consultation and navigation support, working collaboratively with primary care providers to assess and provide specialized cancer care consultation to support care plans and necessary interventions.
