We are currently seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to conduct comprehensive in-home assessments for patients aged 75 and older.
This role is ideal for nurses with experience in home care, in-home assessments, and/or working with the geriatric population.
Position OverviewThe RN or RPN will conduct structured in-home assessments for seniors (75+), with a focus on evaluating safety, cognition, and overall functional status following a geriatric assessment. The goal of this role is to determine the level of safety and support required for patients to remain in their homes.
A comprehensive summary of findings will be prepared and shared with the patient's primary care physician to support further medical follow-up and care planning.
Key ResponsibilitiesConduct in-home safety assessments to evaluate environmental risks and fall prevention needs
Perform cognitive assessments and functional evaluations following geriatric assessments
Assess patients' ability to live safely and independently in their home environment
Identify areas of concern and recommend appropriate supports or interventions
Prepare detailed clinical summaries and reports for the primary care physician
Collaborate with patients, families, caregivers, and healthcare providers
Travel within the assigned region to complete in-home visits
Maintain accurate and timely documentation
Applicants must meet one of the following:
Bachelor of Science in Nursing (BScN) or Bachelor of Nursing (BN)
Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
Preferred Experience:
Home care or community nursing experience
In-home patient assessments
Geriatric nursing or experience working with seniors
Strong clinical documentation and reporting skills
Additional Requirements:
Valid G2/G driver's license and access to a reliable vehicle
Current CPR and First Aid certification
Up-to-date immunization record
TB test completed within the last 6–12 months
Clear Criminal Background Check and Vulnerable Sector Check
Mask fit testing
Proof of COVID-19 vaccination (as required)
Ability to lift up to 50 lbs and assist patients with mobility when required
Strong communication, time management, and critical thinking skills
