Position: Registered Practical Nurse (Hospice)

Responsibilities:

  • Provide direct nursing care to the residents of the Hospice in accordance with the established policies and procedures.
  • Work collaboratively with residents, their families and loved ones, and the hospice team towards achieving identified care goals (as outlined in the plan of care) and the enhancement of quality of life as defined by residents.
  • Complete a comprehensive nursing assessment and documentation that contributes to the resident’s care plan.
  • In collaboration with the RN, recognize actual and potential challenges and identified strengths of the resident and their family and, plan, implement and perform nursing care interventions including ostomy care, wound care, medication administration, subcutaneous injections and ambulatory infusion pumps.
  • Using strong clinical knowledge champion evidence-based practices with particular focus on pain and symptom management at end of life, assessing and treating palliative emergencies and psychosocial distress.
  • Evaluate effectiveness of care plan and make recommendations for changes where necessary.
  • Demonstrate ability to assist residents and family members with concern and empathy; respecting their confidentiality and privacy; communication in a courteous and respectful manner.
  • Identify opportunities for resident participation in decisions that affect the residents’ rights and lifestyle and assist staff in encouraging the residents’ participation in the decision-making process.
  • Act as an advocate to protect and promote the resident’s rights to equitable care, cultural safety, respect, privacy, dignity, and access to information.
  • Consult and collaborate with members of the interdisciplinary health care team about resident’s care.
  • Participate in interdisciplinary team meetings in formulating, delivering and reporting the resident’s daily care needs and preferences as per the plan of care.
  • Participate and support new research projects and initiatives that enhance clinical practice, quality of care/life for each resident and the hospice program.
  • Participate in education and preceptorship of new staff and students.
  • Participate in mandatory education/training in-services for skill development and updates on new and/or relevant information on best practices.

  • Demonstrated ability to work effectively in an interdisciplinary environment.
  • Ability to work well with others in a team approach and adapt to changing situations.
  • Excellent communication skills (written and oral), interpersonal skills and administrative skills that include computer literacy.
  • Willingness and ability to maintain flexibility in the event of unforeseen situations.
  • Disciplinary-free record.
  • Demonstrated commitment to consistent attendance and punctuality.
  • Able and willing to work the scheduled shift.
  • Required Knowledge and

    Experience:

  • Minimum 1 year experience in a palliative care hospice setting preferred.
  • An understanding and commitment to the philosophy of hospice palliative care.
  • Required Professional Designation/Certificate:

  • Minimum diploma/undergraduate degree in Nursing from a recognized institute.
  • Current registration as a Registered Nurse (RPN) with the College of Nurses of Ontario (CNO).
