Job Description

Working as a collaborative member of the inter‑professional team, the Nurse Practitioner (NP) functions in an expanded role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and wellness promotion for adults and seniors.

Senior’s Health Clinics

The NP leads the provision of comprehensive geriatric services in the GAIN (Geriatric Assessment and Intervention Network) Clinic. This is an exciting model of Geriatric Outpatient Care that employs interprofessional comprehensive geriatric assessment (CGA) in the management of frail seniors as well as rapid assessments for our post‑discharge patients from inpatients and ED. The NP provides direct patient care focusing on health promotion, maximizing patient safety and function to support frail seniors living at home.

Outreach

With a growing seniors population in the Scarborough community, supporting our seniors at home is a growing priority for SHN. As an expert geriatric NP, you will provide outreach virtual and/or onsite consultation support to our frail senior patient population in LTC homes, retirement homes and in the community (i.e. home visit as part of GAIN assessment). With a keen ability to obtain critical information provided by care teams in LTC homes, retirement homes and/or in the community, you will assess and provide specialised geriatric consultation to support care plans and necessary intervention.

Consultation

Liaison Services

With growing demands for expert geriatrics services for our inpatient and ED populations, the NP will provide consultation services for frail elderly patients with multiple comorbidities and complex medical regimes in hospital as a member of the consultation liaison service. He/she will provide expert knowledge and support to inpatient and ED teams in the care and treatment of frail elderly patients and work with BSO and psychiatry liaison (CL) team as necessary.

This position is ideal for a Nurse Practitioner with a passion for geriatric care. Hours will require flexibility and will include evening and weekend work, and may include home visits. The successful candidate will be required to provide proof of liability coverage.
