Position SummaryWe are seeking a compassionate and driven Nurse Practitioner to join our new clinic in Toronto. This role is an opportunity not only to provide high-quality, patient-centered clinical care but also to play a foundational role in shaping the clinic’s care model, policies, and procedures as we build a best‑in‐class experience.
Key ResponsibilitiesYou’ll be a trusted clinical partner, providing comprehensive care that supports clients in reaching their health goals while navigating both complexity and simplicity with confidence.
Clinical Care- Deliver full-scope primary care with a focus on prevention and early risk detection
- Diagnose and manage acute and chronic conditions using evidence-informed guidelines
- Conduct comprehensive wellness assessments and annual health exams
- Prescribe medications, perform minor procedures, and provide virtual and in‑person care in the GTA
- Collaborate closely with physicians, RNs, health coaches, diagnostics, and allied health professionals to deliver seamless, integrated care
- Uphold high-touch service standards, ensuring every interaction is grounded in professionalism, discretion, and empathy
- Contribute to the design and refinement of care pathways, workflows, and clinic protocols
- Help foster a culture that supports curiosity, innovation, and shared accountability for client outcomes
- Support the integration of digital tools and data to enhance care coordination and experience
- Master’s-level Nurse Practitioner program (MN‑NP), Family NP designation
- Registered and in good standing with the College of Nurses of Ontario (CNO)
- Minimum 5 years of experience in primary care or family medicine
- Proficiency in clinical assessment, diagnostics, and chronic disease management
- Comfortable working within EMRs and tech-enabled care platforms
- Experience in executive health, concierge medicine, or private care settings
- Familiarity with integrated care models, interdisciplinary teams, or preventive health programming
