Yorktown Family Services is a community-based non-profit social services agency dedicated to providing effective, accessible, quality mental health treatment, prevention, and outreach services to children, youth, women, and families in West Toronto. We are uniquely organized to deliver services through two independent but integrated organizations:
Infant, Child, and Youth Mental Health & West Toronto Youth Hub — a children’s mental health center, and Violence Against Women Services — a shelter for women and their children who are fleeing an abusive relationship.
Primary Function:
The PEERS Coordinator role will provide community-based program developed in response to youth violence prevention support requested from community stakeholders and frontline workers across Toronto.
Responsibilities:- Collaborate with the WTYWH Manager and City of Toronto CDO to support the design, implementation, and continuous improvement of the Community PEERS program.
- Coordinate recruitment logistics and onboarding processes for Peer Support Workers (PSWs), including scheduling and supporting training activities.
- Provide day-to-day guidance and support to PSWs, including pre-briefs, de-briefs, one-on-one check-ins, and assistance during crisis or critical incidents.
- Coordinate and track service requests, ensuring timely assignment (within 48–72 hours) and ongoing communication with relevant partners.
- Maintain accurate program documentation and database records, including regular updates to the TREAT system.
- Support administrative processes, monitoring completion of program plans, summary reports, incident reports, and other required documentation.
- Provide on-site and community-based program support, including flexible scheduling (evenings/weekends) and connecting PSWs to appropriate community resources.
- Assist with program development and service coordination, supporting session planning, referrals, and access to professional development opportunities.
- Support program evaluation and budget implementation, contributing to data collection, reporting processes, financial tracking, and coordination of PSW payments.
- Support stakeholder engagement efforts, including community outreach, referral coordination, placement support, and preparation of interim and final reports.
Qualifications:
- M.S.W. or B.A. in Child Youth Work, Child & Youth Community Services, Social Work, or related field, with 2 years of experience in case management, community engagement or a combined equivalent of education and experience
- Apply practical experience and evidence-informed approaches to support youth development, youth engagement, and community engagement.
- Proven ability in providing individual support, protocol and documentation practices
- Ability to develop and maintain partnerships with relevant youth community resources
- Ability to work remotely and travel between service locations
- Proven training and experience in conflict prevention strategies, mediation, and stress management
- Excellent verbal and written communication and presentation skills in English; second language is an asset
- Service knowledge of the City of Toronto
